The Choate Depression Inventory for Children (CDIC)

Answer true or false.

  • I feel sad lots of the time.

  • I have trouble sleeping.

  • I feel tired lots of the time.

  • I don't have many friends.

  • I cry a lot.

  • I don't like playing with other kids.

  • I don't feel as hungry as I used to.

  • Other kids don't like me.

  • I feel lonely.

  • I have lots of stomachaches and headaches.

  • I don't like school.

  • I have bad dreams.

  • Sometimes I think about hurting myself.

  • I worry a lot.

  • I don't like myself.

  • Other kids have more fun than I do.

  • I don't do as well in school as I used to.

  • Sometimes I have trouble concentrating.

  • I feel angry lots of the time.

  • I get into lots of fights.

If your child answers “True” to three or more items, she should be evaluated by a qualified mental health professional; she may be clinically depressed. And if she answers “True” to Item 13, which reveals self-destructive or suicidal thoughts, she should be evaluated immediately to ensure her physical safety.
