Discharge planning begins at the moment of admission. Years ago, patients would stay in the hospital for days and days, sometimes even weeks or months, but that doesn't happen today. Patients come in sicker and go home sicker than ever before, and they need to go home well prepared. Families and patients have to be ready to take responsibility for care and custodial issues. Nurses need to educate them on how to promote their own wellness.
