This document is a written warning for violation of our call-off policy on Thursday, July 17, 2008.
This is the second time you violated the company's call-off policy. The first occurrence was on Tuesday, June 10, 2008. You received a verbal counseling about this incident on Wednesday, June 11, 2008.
On July 17, 2008, you were scheduled to report to work at 8:00 A.M. You did not report to work, nor did you call. I called your house at 9:30 A.M. to see if you were okay. Your roommate answered the phone and said that she did not know your whereabouts. I called your cell phone afterward and left a message on your voicemail. You called me back two hours later, at 11:30 A.M. Worried about you, I asked if you were okay. You said that you had a cold. I asked you why you did not call me to report that you were unable to work. Your response was that you forgot to call.
This is the second time you have violated the company's call-off policy within the past six weeks. You are aware of the call-off policy. It was brought to your attention on your first day of work during orientation when you received your employee handbook. Additionally, you were reminded of the policy when you received verbal counseling for violating the policy on June 10, 2008.
If you have any questions about what is expected of you when you are unable to report to work, please contact me. It is important that you remind yourself of the call-off procedures next time you are ill. A further occurrence of an improper call-off may result in additional disciplinary action.
