Last-Chance Warning

This is a last-chance warning that one more violation of the company's call-off policy between now and February 26, 2009 will result in the termination of your employment at the ABC Lounge.
It has been documented that you are aware of the company's call-off policy. However, you violated this policy on Tuesday, June 10, 2008 and again on Thursday, July 17, 2008. On both days, you did not contact me by the beginning of your shift to report that you were ill and unable to report to work.
You received a written warning for the improper call off on July 17, 2008. However, you again violated the call-off policy on Monday, August 25, 2008. You were scheduled to report to work at 5:00 P.M., but you were not at your workstation at that time. At 6:00 P.M., you called the switchboard and left a message reporting that you were ill and unable to report to work. I was in my office and had my cell phone with me. You again made no attempt to contact me directly.
To repeat, one more violation of the company's call-off policy between now and February 26, 2009 will result in the immediate termination of your employment. Please contact me if you have any questions regarding the call-off procedures.
