Please be aware that in some circumstances, the FMLA entitles employers for reimbursement of health insurance premiums paid by the employer during leave if the employee does not return from FMLA.

As allowed by section 825.309 of FMLA regulations, please contact your supervisor or the human resource department every four weeks, starting with the beginning of your leave, to give your status and intent to return to work.
