Dear Hourly:
I am pleased to offer you the position of pastry chef at the ABC Lounge in City, State. This job offer is contingent upon the successful completion of a pre-employment physical and drug screen. Please take the enclosed paperwork with you to the occupational medicine clinic located at (address, City, State) on Tuesday, June 2, 2008 at 3:30 P.M. The drug screen is a urine test — please be prepared.
Your starting salary will be $10.50 per hour. As a full-time employee, you are eligible for enrollment in our medical, dental, and life insurance plans. Enrollment materials will be sent to you within two weeks of your start date. If you do not receive them in a timely manner, please let me know. You must sign up for benefits within 30 days of hire to be eligible without delay. You will receive a ninety-day performance evaluation on September, 2, 2008. At that time, you may be eligible for a pay increase of 1%–5%.
