Brief summary of project
Signature of board chair
Federal tax ID and/or other special identity number

Type of organization
Category of grant
Most of the information requested can be prepared in advance. You might want to fill out those portions of the form weeks before you have to submit the package. Following is a sample cover sheet for a foundation grant.
